- USCIS I-9 Form
- Federal Tax Form W4
- Virginia Tax Form W4
- Employee Setup & Direct Deposit Form
- For all employees under the age of 16 – Please fill out a work permit application at the link https://vaeecs.doli.virginia.gov/vaeecs/
