New Employees
New employees must complete the following forms before their first day of work:

Once downloaded and completed please save them to your personal computer, the file name should include your name, and upload them here.
- USCIS I-9 Form
- Federal Tax Form W4
- Virginia Tax Form W4
- Employee Setup & Direct Deposit Form
- For all employees age 14 or 15 – Please fill out a work permit application at the link below. Be sure to use the business name “Dominion Hills Area Recreation Association” in the form https://selfserviceportal.doli.virginia.gov/LaborLaw/ChildLaborPermits/
Current/Returning Employees
